Konoha: Offer your heart to the Hokage

It is the fifth year since Konoha's founding. Hashirama still lives, and the Nine-Tails has yet to be sealed away. Shen, a shinobi specializing in medical ninjutsu, defects from Takigakure to Konoha, bringing with him the secret technique known as Earth Grudge Fear. He goes on to become the hospital's director and leads an ordinary life. Earth Grudge Fear is a secret art that grants near-immortality through the replacement of hearts. Then, as Hashirama prepares to depart for the Valley of the End, Shen steps forward and produces a document he has prepared well in advance—an "Organ Donation Volunteer Form." "Lord Hokage, please sign this before you leave."
